River Valley Services (RVS) is a community mental health center serving towns across Middlesex County, providing high‑quality crisis intervention, case management, residential, and clinical services to adults with serious mental health conditions.
Aboutthe Assertive Community Treatment Team
The ACT team provides intensive, community‑based services to individuals with serious behavioral health and substance use challenges who often struggle to engage in traditional treatment. It is an evidence‑based model that uses a multidisciplinary team approach to deliver highly responsive, individualized, recovery‑focused care in clients’ homes and communities.
Responsibilities- Implement treatment plans for clients on caseloads.
- Provide case management services including crisis response, risk assessment, and daily living skills training.
- Link clients to community supports, medical care, housing, and entitlements.
- Monitor medication compliance and coordinate care with psychiatrists and nurses.
- Deliver crisis intervention and risk management services in community settings.
- Assist clients in navigating community resources with sensitivity to diverse cultural, social, and economic factors.
- Document all client interactions and treatment progress in accordance with the ACT model and accreditation standards.
- Transport clients to and from appointments as needed.
- Administer medication, provide nursing consultation, and contribute to team discussions regarding client care.
- Participate in multidisciplinary team meetings and contribute to case planning.
Proficient knowledge of behavioral health counseling techniques, social work methodology, group work, community mobilization, family dynamics, and psychosocial issues influencing client behavior. Strong interpersonal, oral, and written communication skills and the ability to prepare clear, concise reports and case histories.
Minimum Qualifications – General ExperienceA Bachelor’s degree in a health care field and two (2) years of experience working independently using clinical case management techniques in a behavioral health setting.
Minimum Qualifications – Substitutions Allowed- A Master’s degree in counseling, psychology, or another clinical discipline may be substituted for one (1) year of the general experience.
- A current Connecticut license as a Registered Nurse or a multistate/compact RN license plus two (2) years of experience in a psychiatric setting may be substituted for the Bachelor’s degree.
- A Master’s degree in social work (MSW) and two (2) years at the level of a Mental Health Assistant 2 independently performing full‑range paraprofessional case management may be substituted for the general experience.
- Experience providing case management in a community setting, linking clients to resources, medical care, and entitlements.
