Position Overview:

• Employment Type: Full-Time, Permanent
• Locations: Port Jervis / Middletown, Dutchess County, New York
• Specialty: Family Practice / Geriatric Care
• Schedule: No weekends or calls
• Patient Population: Geriatric / Older Adults
• Base Salary: Up to 170,000 per year

Port Jervis / Middletown, NY – Full-Time NP

• Monday through Friday schedule.
• Provider would work between our Port Jervis and Middletown locations
• Full-time schedule with consistent assigned days/locations

Dutchess County, NY – Full-Time NP

• Monday through Friday schedule.
• Coverage within the Dutchess County area, which may include Poughkeepsie, Wappingers Falls, Beacon, and surrounding locations
• Provider must be comfortable traveling between assigned facilities (although typically never in the same day)

Minimum Requirements:

• Active and unrestricted New York State professional license as an NP
• Active DEA registration
• Active Medicare and Medicaid enrollment
• Current CAQH profile
• For Nurse Practitioners, completion of the required 3,600 hours of collaborative practice experience for independent practice in New York
• Ability to successfully complete our credentialing process
