The Medical Social Worker is responsible for providing social work services to patients in their homes per physician's plan of care.

- Current Ohio License in Social Work
- CPR Certification
Minimum Level of Experience
Required:
1-3 years of job related experience (3-5 years)
Prior job title or occupational experience:
One year experience in a health care setting (hospital, clinic, rehabilitation center, home care, etc.), 3-5 years of experience providing clinical services; experience in home care or community/outpatient setting preferred.

Knowledge / Skills- Computer skills preferred
- Must be capable of standing, stooping, bending and lifting up to 50 lbs.
