Position Overview
The Substance Use Navigator is responsible for providing coordination and support to Premier Health Emergency Department /OB Triage patients and patients admitted to the medical floors who have been identified with substance use disorder (SUD). SUN will support linkage and warm handoff for Medications for Addiction Treatment (MAT) from the ED and inpatient medical floors. This includes performing SUD assessments, linkages to ongoing treatment, community resources, educating patients, communicating with clinical and administrative staff, monitoring and reporting outcomes, and participating in grant‑related meetings and activities.
SUN ensures follow‑up regularly occurs with the identified patient population.
- Coordinate and support SUD patients in the ED and inpatient floors.
- Perform SUD assessments and link to ongoing treatment.
- Link to Medications for Addiction Treatment (MAT) and community resources.
- Educate patients and communicate with clinical and administrative staff.
- Monitor and report outcomes and participate in grant‑related meetings.
- Ensure regular follow‑up with identified patient population.
- Minimum Level
Required:
Associate degree - Type of Degree:
Associate - Area of Study/Major:
Preferred in Allied Health, Chemical Dependency, or Health Services‑related field - Preferred Educational
Qualifications:
Associate Degree in Chemical Dependency, Allied Health, Social Services, or Social Work - Position‑Specific Testing Requirement: N/A
- Chemical Dependency Counselor Assistant (CDCA) or obtain within first 6 months of employment
- Minimum
Required:
1–3 years of job‑related experience - Prior Job Titles/Occupational
Experience:
Community Health Worker, Peer Recovery Supporter, Case Manager, or Emergency Department experience - Preferred Experience:
- Medical terminology
- Emergency department experience
- Substance abuse case management
- Grants
- Other Experience Requirements:
Experience with data entry & Microsoft Office
- Understanding of Substance Use Disorder (SUD) as a medical condition
- Exceptional communication, organizational, and interpersonal skills
- Ability to work with a diverse group of individuals, without judgment, in a culturally relatable way
- Ability to work independently and as an integral member of a team
- Ability to plan, organize, and conduct work with minimal supervision
- Ability to adapt to a changing environment and high‑stress situations
- Ability to design and deliver creative solutions
- Ability to work with confidential patient information
- Ability to conduct motivational interviewing
- Knowledge of available community resources
- Knowledge of EPIC and/or other electronic health record systems
- General computer literacy
