SUMMARY: Provides continuing, comprehensive health maintenance and medical care to the patients of Butler County Community Health Consortium, focusing on culturally competent care for underserved populations. Acts as an advocate for improving healthcare quality and access for all.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Provides primary medical care to the patient population.
• Consults with staff physicians on patient care decisions.
• Evaluates and treats patients in health centers and school-based settings.
• Provides call coverage during nights, weekdays, weekends, and holidays as scheduled.
• Provides patient and family education and counseling.
• May participate in training nurse practitioner students.

Medical Administrative Duties:

• Maintains complete medical records.
• Completes progress notes and patient forms.
• Corresponds with patients and physicians.
• Checks lab reports within 24-48 hours and acts on abnormal results.
• Participates in health improvement activities.

Administrative Duties:

• Performs administrative functions for quality care and accreditation.
• Participates in committees, chart reviews, and quality improvement activities.
• Attends departmental meetings.

Supervisory Responsibilities: This position has no direct reports.

Qualifications:

• Master of Science in Nursing; licensed as a Nurse Practitioner by the Ohio Board of Nursing.
• Two years experience working with indigent populations preferred.
• Bilingual in Spanish preferred.
• Ability to interpret documents, write reports, and solve practical problems.
• Proficient in practice management systems, EMR, Microsoft Word, Internet.
• Strong communication, leadership, and program development skills.

Physical Demands and Work Environment: Must be able to stand, walk, use hands, reach, talk, hear, lift up to 25 pounds, and perform in a moderate noise environment with occasional exposure to fumes, chemicals, and radiation.
