Licensed Practical Nurse (LPN) Position

Centerpoint Health is seeking a LPN to provide clinical and administrative support to our providers and facilitate the delivery of health care services in our health centers located in Norwood, Franklin, and Middletown OH.

• Obtain an individual's basic health status and related data; record information in the electronic medical record.
• Analyze and interpret patient data, make informed judgments, and incorporate the prescribed medical regimen into the nursing plan of care.
• Provide health teaching to individuals and families to maintain optimal health.
• Communicate outcomes of nursing interventions and propose changes in care plans after consulting with physician, advanced practice provider, or RN.
• Collaborate, cooperate, and communicate with other health care providers to ensure quality and continuity of care.
• Follow health center medical and clinical policies and protocols.
• Oversee clinic schedules.
• Document staff successes, concerns, and patient concerns; serve as the front line of conflict resolution with clinical staff.
• Work collaboratively with the Ambulatory Supervisor or Human Resources, in their absence, to address accountability concerns.
• Maximize provider visits to meet productivity targets.
• Close visits with patients, providing education within license restrictions on illness, medical conditions, diet, exercise, medication use, diagnostics, and in‑home testing to ensure good outcomes; review the after‑visit summary.
• Carry out provider orders, including authorization of refills according to established protocols and administering treatments.
• Complete requisitions correctly, send specimens to the correct laboratory, and perform accurate lab linking and medical necessity notice filings.
• Follow insurance guidelines for preauthorization, diagnostic scheduling, referrals, and specialty visits.
• Schedule diagnostics or specialist visits within network guidelines as ordered by the provider.
• Other duties as assigned.

• Maintain an unrestricted LPN licensure in Ohio.

• Prefer 3 years as an LPN providing direct patient care in a health care center.
• Primary care experience preferred.
• Proven customer service experience required.

• Patient‑centered medical home (PCMH) experience and/or community health center experience.
• Attention to detail and ability to follow directions independently as appropriate.
• Interpersonal skills required to work collaboratively with clinical staff of all levels.
• Ability to assist in optimizing the delivery of patient services, including knowledge of the scope of practice of the clinical team.
• Bloodborne Pathogens Exposure:
  Category I.

• Must be able to lift up to 25 lbs. and perform bending, pushing, pulling, and squatting as needed.
