Overview

Sub-Acute, Long-Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility, with no on‑call duty. We are seeking a Fee‑For‑Service Attending Physician/Medical Director for our Grand Islander Center, located in Middletown, RI. This dual‑role position is clinical in nature and requires a board‑certified Internal Medicine or Family Medicine physician with long‑term care experience.

Align Med Partners is a national medical group dedicated to improving health outcomes among residents of skilled nursing and senior living facilities. Our physicians, nurse practitioners, and physician assistants are supported by a clinical leadership team with a decades‑long track record of excellence in post‑acute and long‑term care.

Responsibilities
  • Work with the Center Medical Director to ensure delivery of high‑quality clinical services aligned with Align Med Partners mission.
  • Oversee implementation, utilization, and outcome measurement of Align Med Partners Clinical Programs and nationally accepted standards of care.
  • Report directly to the Align Med Partners Regional Medical Director and coordinate with the Center Administrator and Medical Director.
  • Take complete responsibility for patients admitted to the center, including timely visits, comprehensive history and physical examinations within 72 hrs, and high‑quality care delivery.
  • Adhere to Align Med Partners credentialing process, policies, and procedures, including “Rules and Regulations” of the Medical Staff and “Corporate Integrity” policies.
  • Comply with all relevant state, federal, and institutional regulations (False Claims Act, HIPAA, etc.) and participate in required orientation and training.
  • Report non‑compliance concerns promptly via the Reach Out Line and cooperate with monitoring and audit functions.
  • Perform additional duties as assigned.
Qualifications
  • M.D. or D.O. required.
  • Board‑certified in Internal Medicine or Family Medicine;
    Board‑eligible Preference.
  • Maintenance of certification required.
  • Geriatrics specialization desirable.
