Schedule

This is a full time, hybrid position, 40 hours per week. The schedule is flexible and can be Monday - Friday 7:30am - 4:30pm or Monday - Friday 8am - 5pm.

Job Summary

Responsible for participating in an interdisciplinary assessment regarding individuals seeking or residing in nursing home level of care to determine the least restrictive living environment to meet their overall needs.

Responsibilities and Duties

Assesses OBRA/PASARR referrals assigned by OBRA Coordinator and/or designee. Provides education as needed on the OBRA/PASARR process to community partners, consumers, and families. Completes timely assessments for consumers seeking nursing facility placement or already in nursing facility placements. Ensures consumer placed in appropriate and least restrictive living environment to meet their mental and physical health needs. Provides back‑up coverage for OBRA therapists in completing Level II OBRA screens for team leave requests.

Ensures Specialized Mental Health services are being provided as authorized within our six‑county catchment area. Has an acute knowledge of the Level of Care Determination (LOCD) requirements for nursing facility placements. Ensures compliance with MDHHS‑OBRA regulations concerning timelines for all assessments. Completes reviews and delivery of Level II evaluations to the individual, legal guardian, hospital, and nursing facility as required by MDHHS‑OBRA.

Attends OBRA related team meetings and/or trainings related to professional development and or improving practices. Participates in peer reviews or interdisciplinary meetings prior to submission of evaluations. Employment Standards
  • Education: Master’s Degree is required with a major in Social Work, Counseling or Clinical/Counseling Psychology.
  • Job‑Related

    Experience:

    Sufficient experience to perform the position at a satisfactory level required. Experience with relevant population in a community mental health setting preferred.
  • Licenses and

    Certifications:

    Michigan state licensure. Valid driver's license while operating vehicle on agency business.
Conditions of Employment Knowledge

Knowledge of Medicaid Guidelines, OBRA rules and regulations, PASARR, and Level of Care Guidelines. Knowledge of eligibility standards and an understanding of service programs, purposes and elements; knowledge of entitlement programs, eligibility requirements and benefits, use of community resources and services. Knowledgeable about types and levels of available community housing options, including but not limited to nursing facilities, adult foster care, assisted living, and or private homes and what community resources are available to support those in private homes.

Knowledge of developmental disabilities, mental illness, and neurocognitive disorders.
