Overview
Longevity Health is looking for a Nurse Practitioner or Physician Assistant with a passion for working with seniors in long-term care settings. In this role, you'll be responsible for a panel of enrolled members receiving nursing home care. You'll be supported by a clinical leadership team that provides education and guidance on best practices for effectively caring for our members. As part of your role, you'll conduct comprehensive assessments and develop individualized care plans for each member.
You'll participate in nursing facility care team meetings, maintain frequent communication with members' families, and ensure the care team stays aligned with members' goals and needs.

Responsibilities- Provide collaborative primary care and services to a panel of Longevity Health members, selecting the most appropriate setting based on the member’s goals of care.
- Execute and deliver the contracted Plan Model of Care activities, develop and implement advanced care planning, and promote the treating-in-place model.
- Facilitate and administer health plan clinical operations functions—including prior authorization, Part A and B services, medication reviews, and quality gap closure (HEDIS and STARs)—to promote membership growth.
- Serve as a trusted advisor to members, families, and nursing home staff; educate care teams on member needs and early identification of changes in condition; notify APP and telehealth partners for member changes.
- Deliver effective, frequent communication and coordination with members’ PCPs, families, and care team members on care needs, goals, and treatment plans/changes.
- Support overall market goals and balance member caseloads with team member support needs.
- Promote Longevity Health with members, families, nursing facilities, and other stakeholders.
- Function autonomously and be accountable for practice; establish meaningful relationships in the nursing home environment; maintain member-centric, high-quality care.
- Perform other related duties as assigned.
- There are no supervisory responsibilities for this position.
- State unrestricted licensed Advanced Practice Nurse with National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner; or an equivalent sanctioned by state; will allow acute care nurse practitioner with nursing home/geriatric or hospice/palliative experience, OR state unrestricted licensed Physician’s Assistant NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
- Active DEA license in the state of practice is required.
- Active national provider number (NPI) and an active Medicare provider number.
- Eligible to participate in the Medicare program as a provider and registered.
- Valid state driver’s license with a good driving record and proof of automobile insurance; auto insurance coverage required by state of residence. Willingness to drive up to one hour to and from work daily.
- Must be present in SNF communities/buildings during business hours daily.
- Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program.
- Minimum of 6 months experience as a Nurse Practitioner/Physician Assistant; experience with geriatric/hospice or palliative care preferred prior to NP/PA; or licensed and board-certified new graduate NP/PA in Longevity Health’s New Graduate Fellowship program with competencies for independent practice.
- Experience working in a nursing home environment strongly desired.
- Computer/technology proficiency; ability to engage and communicate effectively with physicians, patients, and families.
