Heartland Hospice, part of Gentiva, is seeking an After hours RN Care Manager to provide high-quality, compassionate hospice care outside standard business hours in Michigan. You will oversee clinical care for patients and coordinate with the IDG team during nights, weekends, and on-call shifts.

Responsibilities include direct patient assessments, admissions, care plan development, education for families, and timely documentation, while maintaining regulatory compliance and supporting field staff
