• The RN Case Manager plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience.
• The RN Case Manager coordinates and collaborates with other hospice team members to meet the physical, psychosocial and spiritual needs of the patients and their families. This is an exempt position.

• Registered Nurse (RN) with experience in hospice care or community health education/experience.
