- Plan, implement, coordinate, evaluate, and promote continuity of care in accordance with established standards and practices.
- Assess health status through patient interviews and review medical history.
- Evaluate patient/family needs and provide teaching and instructional materials.
- Offer psychosocial support to patients and families.
- Facilitate clinical operations and patient care.
- Manage medication refill requests from the pharmacy line and document in the patient chart.
- Schedule consultations, pre‑testing, diagnostic tests, procedures, referrals, and future appointments.
- Triage patient phone calls as needed and document relevant information.
- Coordinate communication between patient/family and the interdisciplinary team.
- Assist patients in locating community resources.
- Collaborate with inter‑professional team members.
- Serve as a resource to LPN/LVNs and medical assistants.
- Graduate from an accredited nursing program; BSN preferred.
- Minimum of one year of nursing experience.
- Willing to consider entry‑, mid‑, or senior‑level candidates.
- Current RN license in the state of practice.
- Current Basic Life Support (BLS) certification.
The work environment may include exposure to communicable diseases, toxic substances, ionizing radiation, and medical preparations, typical of an oncology/hematology clinic. Reasonable accommodations are available for individuals with disabilities.
Physical RequirementsFull range of body motion, including lifting and handling patients, manual and finger dexterity, and eye‑hand coordination. Required to stand and walk for extended periods and occasionally lift up to 40 lbs. Vision and hearing must be within normal limits.
