Our team of clinicians breaks through traditional barriers to care, ensuring that older adults and at-risk individuals receive the attention they deserve.

As a Nurse Practitioner or Physician Assistant, you will:

• Conduct thorough assessments of adult and geriatric patients, including medical history, diagnosis, treatment, and health education.
• Administer and order necessary point-of-care and lab-based testing.
• Collaborate with physicians, case managers, and caregivers to provide holistic care.
• Deliver services directly in patients' homes or skilled nursing facilities.

To thrive in this role, you should have:

• A Master’s Degree or equivalent experience with NP or PA licensure.
• Current NP/PA licensure in your state, including prescription authority.
• Board certification in relevant specialties (NCCPA, AANP, etc.).
• Current BLS, ACLS, or CPR certification.
• 1 year of NP or PA experience preferred, but new grads are encouraged to apply!
• A valid driver's license and proof of automobile insurance.
• Strong communication skills and proficiency in medical record software.

• Competitive Pay: Annual base salary ranging from $95,500 - $158,400 .
