Position Summary

St. Anthony’s Hospital in Midwest City is seeking a dedicated and compassionate Case Manager to join our System Case Management team. This weekend-only position is ideal for professionals looking to make a meaningful impact while enjoying a consistent and focused schedule.

Department: System Case Management
Schedule: 8am-2pm (any four days between Monday and Friday.)

• Coordinate patient care and discharge planning
• Collaborate with interdisciplinary teams to ensure quality outcomes
• Advocate for patient needs and connect them with appropriate resources
• Maintain accurate documentation and compliance with hospital policies

• RN or relevant licensure/certification in case management or social work
• Experience in hospital or healthcare case management preferred
• Strong communication and organizational skills
• Ability to work independently during weekend shifts
