Social Worker PRN – Caris Healthcare

Caris Healthcare is hiring a Social Worker PRN to provide psychosocial and emotional support to patients and caregivers at the end of life. The role is part of the hospice interdisciplinary team and involves comprehensive patient assessments, care planning, and bereavement support.

Pay: BSW $22.50 – $26.50 / hr. | MSW $25.50 – $31.25 / hr. (depending on experience)

• Complete and document comprehensive psychosocial assessments in the electronic medical record.
• Identify care planning goals and develop interventions to promote best outcomes.
• Participate in Interdisciplinary Team meetings and provide insight into psychosocial and emotional needs of patients.
• Provide education related to funeral planning and advanced directives as needed.
• Conduct visits with the patient and caregiver in their residential setting.
• Assist with bereavement support and interventions for caregivers following the death of a patient.
• Participate in an on‑call rotation for after‑hours visits when needed for a patient’s death, funeral, admission, etc.

• Tennessee and Missouri: MSW preferred, BSW required.
• Virginia: MSW preferred, BSW required.
• South Carolina:
  Social Work Licensure required; LMSW required.
• Georgia:
  Social Work Licensure required; LMSW preferred, LBSW required.
• One (1) year of social work experience in a health care setting.
• Excellent communication and interpersonal skills.
