Position: Sister / Charge Nurse Community

This is a Fixed Term/Secondment vacancy until 30th May 2027 to cover maternity leave.

As a Band 6 SPQ Community Nurse you will provide high‑quality nursing care to patients with complex health needs in the community. You will assess, plan, implement, and evaluate care, helping people remain independent and avoid unnecessary hospital admissions.

This vacancy is a part time 30 hours per week post. Hours will cover a 7 day service which covers .

Main duties of the job

The role involves managing a caseload, supporting junior staff and team leaders, and working closely with the wider multidisciplinary team and third sector. You will work closely with specialist nurses, social care and secondary care.

You will undertake comprehensive patient assessments, develop and review individual care plans, deliver and coordinate nursing interventions in the patient’s home.

Other duties include managing patients with complex needs, chronic, palliative and end‑of‑life care, performing and overseeing wound care, catheter care, medication and prioritising visits according to clinical needs.

You will coordinate care across multiple agencies and services, acting at all times as a role model and clinical resource for band 5 nurses, assistant practitioners and health‑care support workers.

Provide supervision, mentoring and teaching, support staff development, competencies assessments and contribute to service improvement initiatives.

The ability to speak Welsh is desirable for this post;
