Brandywine Counseling and Community Services is seeking a part-time Community Wellness Advocate for our Milford location!

We promote hope and empowerment to persons and families touched by mental illness, substance abuse, HIV and multiple occurring diagnoses, and their related challenges.
The Community Wellness Advocate has primary responsibility for providing quality, sufficient and appropriate prevention, advocacy, and treatment services in the community
What You’ll Do as a Community Wellness Advocate:- Help individuals navigate the health care and/or social service systems, enabling them to receive primary and preventative care, maintain healthy behaviors, and manage chronic conditions in culturally and linguistically relevant ways
- Community-based HIV counseling and testing
- Risk reduction education sessions
- Provide culturally appropriate health education
- Educate and counsel clients about plans of care, health promotion, and disease prevention
- Educate client and colleagues on features of health systems
- Meet with clients as needed to address wellness in settings conducive to the client
- Provide individualized education customized to the client and support system.
- Refer clients to appropriate clinical and/or medical staff
- Participate in treatment planning meetings
- Conduct individualized risk and needs assessments
- Provide resources and coordinate referrals
- Provide transportation for appointments. Accompany clients to appointments and assist with information dissemination. Aids in educating clients and support systems pertaining to information received at appointments
- Work directly with the counseling team to develop an individualized plan of care
- Educates client to non-pharmacologic approaches to disease and symptom management
- Document all interactions with clients including any assessment findings, physical/psychosocial responses to interventions and progress towards problem resolution. Reports any changes in client’s condition to appropriate personnel.
- Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the client’s diversity, preserving their autonomy, dignity and rights.
- Case Management, Outreach, and Community Health Worker practices as they relate to best practice and BCCS activities.
- Outreach and prevention requirements, including Federal and State regulations, as well as contract requirements.
- Behavioral health principles, standards, public health, wellness principles, advocacy principles and best practices.
- Knowledge of local community resources

- REQUIRED
:
High school diploma or GED - REQUIRED
: 2-years' experience in the human services field - REQUIRED
:
Valid Driver's License with 2 Points or Less - PREFERRED: CWH certification

- Monday – Friday
- 9:00 a.m.
