Mid Furness and Millom Integrated Care Community Case Manager Job Overview

Band 6 case manager role in the Mid Furness and Millom Integrated Care Community (ICC) Team. Working hours Monday to Friday, 08:00‑16:00. Base to be determined within the Mid Furness and Millom ICC footprint.

Ideal for experienced Band 5 Community or District Nurses, or experienced Band 6 practitioners wishing to broaden experience within the integrated care model.

Main responsibilities
  • Provide holistic, evidence‑based care to individuals with complex needs, undertaking comprehensive assessments and reviews within patients' homes and community settings.
  • Manage a complex caseload of patients with long‑term conditions, frailty and multiple health and social care needs.
  • Coordinate care across multidisciplinary teams to ensure seamless and person‑centred service delivery.
  • Support patients to remain independent at home through proactive care planning and intervention.
  • Develop, implement and evaluate personalised care plans in partnership with patients, carers and professionals.
  • Provide clinical advice, guidance and signposting to appropriate services and support networks.
  • Lead holistic care planning and promote shared decision‑making.
  • Support proactive case‑finding and population health management initiatives.
  • Monitor patient outcomes and contribute to continuous quality improvement.
  • Act as a role model and mentor for colleagues, supporting learning and professional development.
  • Contribute to service development and the ongoing evolution of the ICC model across Mid Furness and Millom.
Qualifications
  • BSc (Hons) or BNurs (Hons) Nursing Degree, or equivalent, or BSc (Hons) Paramedic Science Degree.
  • Active NMC or HCPC registration.
  • At least 2 years of community experience.
  • Excellent communication skills, both written and verbal.
