- Pay Scale $80,000-$90,000 annual salary
The Registered Nurse (RN) will provide care and services for a group of patients, visiting patients in private homes, skilled nursing facilities, and assisted living facilities. The RN is a contributing member of the interdisciplinary team, offering expertise in psychosocial assessments and interventions, and knowledge of community resources. The RN will work in either a Case Management, Admissions, or Resource role.
The RN works independently on a day‑to‑day basis, handling phone, email, written communication, meetings with a variety of healthcare professionals, and understanding hospice rules and regulations. Time‑management skills are required to schedule visits, document care, attend meetings, and triage urgent needs.
Qualifications- Current registered nurse (RN) licensure in good standing in the state(s) of practice
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
- Works closely with patients on a 1:1 level with strong support from the patient care team
- Can work independently and manage phone, email, and written communication; attends meetings with a variety of healthcare professionals
- Time‑management skills to manage visits, documentation, and urgent needs by prioritizing and triaging multiple concerns
- Ability to learn an electronic medical records system
- Willingness to drive with reliable transportation, valid driver’s license, and auto insurance, to travel across the service area
