Base pay range: $25.00/hr - $30.00/hr

Milton Freewater Health and Rehabilitation Center is currently seeking a Social Services Director to assist with maintaining the social service program designed to meet the individual physical, mental, and psychosocial needs of the residents.

• Works with residents, families, and community support to complete social service history, discharge plan, and development of interdisciplinary care plans designed to meet the bio-psychosocial needs of residents.
• Assists residents and responsible parties in locating and using financial, legal, mental health, and other community resources.
• Coordinates and/or provides health and mental health social work services to residents to assist with attaining or maintaining the highest practical mental and psychosocial well-being, while helping residents who display mental or psychosocial difficulty receive appropriate treatment and services.
• Provides resource support to staff participating in behavioral interventions for residents.
• Coordinates safe discharge planning and follow‑up services in coordination with interdisciplinary team.

• Effective communication skills with residents, families, and staff.
• Preferred: a minimum of Bachelor’s Degree in an accredited social work program.
• Preferred: experience in a long‑term care setting.
