The Social Prescriber will work as part of the multidisciplinary PCN team to support patients with non-medical needs that impact their health and wellbeing. The role involves person-centred conversations, identifying what matters to people, and connecting them to community-based support, voluntary services, and local resources across the PCN footprint.
The post holder will work across five GP practices, ensuring equitable access to social prescribing services for patients and supporting population health priorities.
Main duties of the jobKey Responsibilities
- Receive referrals from GPs, practice staff, and the wider PCN MDT
- Conduct holistic assessments using a person-centred, strengths-based approach
- Co-produce personalised care plans with patients
- Support individuals to address issues such as:
- Social isolation and loneliness
- Mental health and wellbeing
- Financial, housing, or employment concerns
- Lifestyle changes (e.g. physical activity, weight management)
- Provide follow-up support to help patients engage with services and build confidence
- Build and maintain strong relationships with local VCSE organisations, community groups, and statutory services
- Maintain an up-to-date directory of local services across the PCN area
- Promote community assets and support services within GP practices
- Act as a link between primary care and the community sector.
- Work flexibly across five GP practices, attending MDT meetings as required
- Contribute to care coordination for complex patients
- Support PCN priorities such as:
- Health inequalities
- Long-term condition management
- Mental health and wellbeing
- Prevention and early intervention
Nexus MK PCN offices are based in Linford Wood Milton Keynes and cover 5 practices which include Oakridge Park Medical Centre, Wolverton Health Centre, The Stonedean Practice, Sovereign Health Centre, Purbeck Medical Centre.

Data, Monitoring & Reporting
- Accurately record patient contacts and outcomes using clinical systems (Systm One)
- Collect outcome data aligned to ARRS and PCN reporting requirements
- Contribute to audits, service evaluation, and continuous improvement
- Ensure compliance with information governance and confidentiality standards
Professional Practice
- Work within the NHS Social Prescribing Model and PCN DES requirements
- Maintain professional boundaries and manage risk appropriately
- Engage in supervision, training, and continuous professional development
- Promote equality, diversity, and inclusion in all aspects of practice
