Home 1st Planned Care is looking for a motivated, compassionate Band 3 Rehabilitation Assistant to join our forward‑thinking team in Milton Keynes. This is more than a job. Clinical Duties- Provide physiotherapy, occupational therapy and nursing interventions as part of a specific treatment plan agreed and documented by physiotherapist, occupational therapist or nurse.
- Undertake tasks in delivering patient care, rehabilitation or social care as directed by the service professionals in a range of settings.
- Work independently in the designated clinical area or patients’ home following an agreed treatment plan.
- Provide support work with patients individually or in a group with or without supervision of a physiotherapist or occupational therapist.
- Follow departmental and trust guidelines for obtaining consent and use diplomacy and gentle encouragement to enhance patient compliance in treatment programmes.
- Work with family members, carers and support workers to enable them to carry out treatment programmes planned by the physiotherapist, occupational therapist or nurse.
- Manage own workload allocated by the physiotherapist, occupational therapist or nurse.
- Work as a member of the multidisciplinary team and liaise with the team effectively to ensure a co‑ordinated approach to patient care; attend meetings as delegated by the nurse, physiotherapist or occupational therapist.
- Maintain comprehensive and accurate records of work undertaken, including documenting in line with Trust and service standards.
