Main duties of the job Identify the training needs of Primary care clinical staff. In conjunction with the H&J Lead Nurse and the Trust training department, develop a training programme which addresses the needs of staff.

Act as a resource on primary care delivery, providing information and support to staff.

Line management responsibility for Band 6 staff including appraisal and performance management

Encourage and facilitate staff in developing skills and knowledge of appropriate interventions for their patients.

Manage and provide supervision and training to staff in line with Trust policies.

Ensure appraisals are up to date and each member of the team has a personal development plan.

Develop clinical skills and systems to allow for effective triage, minor aliment management and appropriate referral to other healthcare services.

Participate in the induction and training of new staff, peers, junior staff and students.

Undertake mandatory training appropriate to healthcare and prison environment and ensure maintenance of fitness to practice.

Ensure that mandatory training is kept up to date and request training from line manager if further support required.

Ensure that any training undertaken is evaluated on completion and that skills acquired are utilised to enhance team performance and delivery of care.

Ensure individual performance and development objectives are met in accordance with the service aims and objectives.

Job responsibilities

Plan, deliver and evaluate interventions for treatment as clinically appropriate using evidence-based models

Set up processes/protocols for effective assessment, planning, implementation and evaluation of care for all patients with a range of physical health care needs, including those who may also have mental health and/or substance misuse needs.

Safeguard vulnerable patients in line with Trust safeguarding policies. Provide brief interventions - health education, advice and information - to patients and develop Health promotion programmes to be delivered in partnership with the prison.

Provide specialist clinical advice, training and support to colleagues on the management of patients.

Apply local Risk Assessment and management protocols.

Make appropriate referrals to other services for long term or complex care management e.g. detox, counselling, psychology.

Advise Primary care clinical staff and GPs on medication issues, promote adherence to local medication policies/protocols and compliance with legal and professional standards. Possess knowledge of the security implications of medicines within Health & Justice Services.

Monitor the on-going health status of patients and address identified  multi-disciplinary clinical practice to provide focused, integrated care and treatment programmes.

Work collaboratively with primary care staff to develop clinical services that deliver high quality clinics e.g. minor ailments, chronic disease clinics and health promotion initiatives.

Use highly skilled interventions to de-escalate aggressive patient behaviours and be aware of the appropriate processes for dealing with them.

Work collaboratively with staff of other disciplines e.g. prison, education staff to provide integrated and coordinated care.

Work within own sphere of competency and ensure that training needs are communicated to the team manager.

Lead local audits and participate in trust audits to enhance quality of care.
