An incredible opportunity has arisen for a Band 6 Community Mental Health Practitioner specialising in mental health to join the dynamic and dedicated team at Fairfield’s House in Brent, London. This role offers a competitive rate of £27 per hour, working full-time at 37.5 hours per week. The duration of the vacancy is to be confirmed, with a start date as soon as possible and an end date on 09 November 2026.

What you will do

  • Be a crucial part of the Brent Integrated Community Mental Health Team, leading on primary care mental health liaison and link working.
  • Conduct initial and comprehensive assessments of health and social care needs, both independently and with colleagues, and present your findings to the Multi-Disciplinary Team.
  • Coordinate care for an identified group of service users, ensuring a recovery-focused and inclusive approach as a Care Coordinator under the Care Programme Approach.
  • Provide supervision to a team of staff and students on placement, fostering a nurturing and educational environment.
  • Participate in Multi-Disciplinary Team activities, including duty and clinic rotas, enhancing the team’s overall efficiency and service delivery.
  • Develop and maintain effective communication strategies between mental health services and general practice teams, streamlining care pathways.
  • Ensure adherence to CNWL policies and procedures, upholding the values and ethos of the organisation.
  • Operate within the professional frameworks and legal guidelines, such as the Professional Conduct Code, Adult and Child protection protocols, and the Mental Health and Capacity Acts.
