The Frailty SDEC Clinical Nurse Specialist plays a key role within the Acute Assessment and Frailty Team (AAFT), delivering high quality patient‑centred care to older people presenting as emergencies to Milton Keynes University Hospital. The post holder will work autonomously and as part of a multidisciplinary team to provide comprehensive geriatric assessment, support timely clinical decision‑making, prevent unnecessary admissions, reduce length of stay, and enable safe and effective discharge across the emergency care pathway, including ED and Same Day Emergency Care (SDEC).

We are looking for an experienced, motivated nurse with strong clinical assessment skills, a sound understanding of frailty syndromes, and the ability to work confidently in a fast‑paced acute environment.

Main duties of the job

The ideal candidate will demonstrate excellent communication skills, advanced clinical reasoning, and the ability to collaborate effectively with medical, therapy, and community teams. Experience in frailty, acute medicine, emergency care, or complex discharge planning is essential, alongside a commitment to compassionate, dignified care for older people.

This role offers significant opportunities for professional growth, including development of advanced frailty expertise, leadership within the frailty pathway, involvement in service development, quality improvement, audit and research projects, and delivery of education and training across the Trust. The post holder will be supported with ongoing learning, appraisal, and career progression, with scope to develop advanced clinical, leadership, and service improvement skills within a growing and innovative Frailty SDEC service.

Post is subject to an initial probationary period of six months, extendable to 12 months. A DBS check may be required for new employees.

Qualifications and knowledge
  • ENB 998 or equivalent
  • Diploma/Degree in Nursing
  • History Taking and Patient Assessment course
Experience
  • Post registration experience in medicine or geriatrics
  • Established experience in intravenous therapy, venepuncture and cannulation
  • At least 3 years previous experience at band 5 and senior staff nurse level in medical or surgical setting
  • Experience/appreciation of community issues/needs
  • Experience in managing patients with frailty and CGA
  • Knowledge and understanding of Same‑Day Emergency Care Pathways
  • Coordinating the shift in the absence of the Senior Sister
