MK Primary Care Network has exciting opportunity for an advanced nurse practitioner or an advanced paramedic practitioner to join our practice team. The role is offered on a full‑time basis, 37.5 hours per week over 5 days per week, between Monday and Saturday, on a rotation basis across the 4 surgeries - Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice and The Grove Surgery that make up the PCN.

The post-holder will act as an autonomous advanced practitioner able to work independently as well as in conjunction with other members of the clinical team. The post-holder will be an experienced, caring nurse who can demonstrate highly developed clinical skills and is able to demonstrate critical thinking, whilst working within their professional boundaries. They will be able to demonstrate safe, competent clinical decision‑making and deliver expert care, including assessment, diagnostic skills, treatment and evaluation of patients who wish to be seen by a healthcare professional, presenting with minor illness and/or their continuing medical and nursing needs.

• Acting as an expert practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.
• Working as an autonomous clinical practitioner/nurse, providing high quality and timely patient centred care.
• Independently prescribe, working within the current guidelines, reviewing, and monitoring patient medications in accordance with the formulary.
• Delivering face to face/telephone/e-consult/video consultations as well as home visits.
• Directing care appropriately and managing complete episodes of care independently.
• Undertaking consultations with patients of all ages including children as an autonomous practitioner and using own clinical judgement, diagnose, treat, refer and/or discharge patients.
• Working within and across the MDT teams and signpost/draw upon expertise of all members to support and meet the patients' needs and best interests.
• Recording clear and contemporaneous consultation notes to agreed standards, ensuring accurate read coding.
• Participating in, and contribute to, all clinical targets (QOF, QIS, ES etc).
• Recommending, instigating, and explaining appropriate invasive and non-invasive diagnostic tests, investigations and interpret findings and reports.

East:

MK PCN covers a total population of around 56,000 patients, across its 4 practices in Milton Keynes:
Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice and the Grove Surgery. Our overarching objective is to deliver the highest standard of care.
