This pivotal position demands a commitment of three weekdays plus one in three weekends, blending community care and clinical excellence. You will engage in a range of responsibilities including wound care, IV therapy, and palliative care.
Key Responsibilities:
• Provide clinical skills in home and community settings
• Collaborate with interdisciplinary healthcare teams
• Apply person-centered care approaches
• Support ongoing development through mentorship
• Navigate patients to additional health resources
Requirements:
• Must be a Registered Nurse in Ontario
• Criminal Record Check with Vulnerable Sector Screen
• Valid driver's license and insured vehicle
• Professional Liability Insurance required
• Strong English communication skills
