In this part-time nursing role, you will visit patients within their homes, promoting comfort and well-being. Key Responsibilities:
• Deliver nursing care in community settings
• Assess individual patient needs and deliver personalized care
• Monitor and adjust care plans accordingly
• Develop trust and rapport with patients and families
• Maintain comprehensive, accurate patient records
Requirements:
• Current CNO registration as a Registered Practical Nurse
• Possession of a valid driver’s license and vehicle
• Community care experience preferred
• Flexibility and reliability for patient visits
• Willingness to provide after-hours on-call support
