• 7 PM, every other weekend).

** Responsibilities:*
* • Provide exceptional nursing care to hospice patients.

• Document using HCHB and OASIS, ensuring same
• day completion.

• Collaborate with a multidisciplinary team on care plans.

• Float within the facility as needed.

* • Active RN License and BLS certification.

• 2+ years in inpatient hospice care.

• Proficient in HCHB/OASIS, with Oasis E experience post
• January 2023.

• Manage productivity points, averaging 26
• 28.

• No more than a 1
