Job SummaryThis position encompasses the care of people dealing with and recovering from mental illness. Under supervision, the case manager will develop treatment relationships with clients and create and implement treatment plans with the necessary follow‑up, case management, and interventions.
Essential Functions- Develop treatment relationships with clients and formulate treatment plans outlining the services to be provided.
- Provide psychosocial rehabilitation, symptom monitoring & management, and supportive counseling to clients.
- Monitor clients’ psychiatric medication regimens and compliance as directed by physicians.
- Provide crisis intervention and supportive counseling to clients in crisis.
- Maintain a treatment‑oriented environment under program supervision and ensure consistency in carrying out treatment objectives.
- Coordinate utilization of community resources, day programs, and outside health care providers.
- Assist clients with financial planning and income resource development.
- Coordinate client access to services and entitlement programs.
- Provide treatment‑related education and liaise with client families.
- Assist clients in securing adequate housing and train them on skills needed to maintain housing.
Minimum Qualifications
- Bachelor’s Degree in behavioral science required, preference for social work experience;
Master’s degree encouraged. - Meet all WCS requirements for criminal and caregiver background checks, references, and driving record.
- Valid driver’s license with automobile and sufficient insurance to meet agency requirements.
- Good computer skills (Word, Excel, CSP Exchange).
- Strong organizational skills and ability to multitask and meet deadlines.
- Knowledge of mental illness and ability to work with individuals diagnosed with mental illness.
- Ability to work independently and as part of a team.
Work is performed largely in the community, including apartments, client homes, community locations, and other settings. Hours of work are during regular business hours but may vary depending on client and program needs, averaging at least 40 hours a week.
Physical DemandsPosition is mobile with time spent in community, including transporting clients, home visits, and office work; driving throughout Milwaukee County; must be able to go up and down stairs and lift/carry up to 20 pounds.
