* Position Summary:

** The Complex Care Program provides care coordination and co-management for children with medical complexity due to multiple chronic conditions. The role will meet with patients and families to identify barriers to coping with acute or chronic illnesses/disabilities, while also providing individual, group and family support to patients and their families enrolled the Special Needs program, both in an inpatient and outpatient setting.

The role provides crisis intervention as well as long-term therapeutic support for the entire family system.

The role will assess, plan, and implement support plans to meet the overall needs of the entire family.  The role empowers patients/families to utilize resources to ensure necessary medical care and to achieve optimal psychosocial functioning.  The role will implement and maintain supportive programing within Complex Care to serve the entire family, including but not limited to sibling groups, educational opportunities, inpatient groups and family events.  

This role will also assist with management of the Family Leadership Committee as well management and educational support of Trauma Informed Care within the Complex Care Program.
** Position Requirements:
*** Master’s degree in social work or related field, plus 3000 hours post Master's experience or PhD or PsyD from an APA accredited program.
* State of Wisconsin Licensed Clinical Social Work (LCSW), Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT) licensure required.
* Three or more years of experience working with pediatric patients. Knowledge of psychotropic medications.
* Experience in working with children and youth with special health care needs and their families preferred. Ability to provide quality individual, group and family therapy.
