Looking for RN's in the Milwaukee, Sheboygan & Central Wisconsin areas with some Hospice experience!
Schedule- Milwaukee areas: Hourly position up to $45/hr, Monday–Friday 1st shift.
- Sheboygan areas: Hourly position up to $45/hr, Monday–Friday 1st shift. Eligible for $5,000 sign‑on bonus.
- Central WI areas: Hourly position up to $45/hr, Monday–Friday 1st shift. Expanding into Central WI.
Responsible for planning, organizing, developing, and directing the nursing care of the hospice patient in accordance with current federal, state, and local standards, guidelines, and regulations.
Essential Functions- Assist the admission nurse in coordination of timely admissions.
- Provide hands‑on clinical nursing, interacting with patients, family, physicians, Interdisciplinary Team members, and other healthcare professionals involved in the patient’s care.
- Assess and coordinate a pain/symptom management program formulated with participation of family, patient, attending physician, and the Interdisciplinary Team, utilizing pharmacologic and non‑pharmacologic methods to enhance quality of life.
- Assist in facilitating freedom of choice, relief of suffering and death with dignity for patients facing end‑of‑life issues.
- Educate patient and family on disease process and expectations related to disease progression.
- Provide support and education for families based on the hospice philosophy of care, including physical, psychological, spiritual, and financial comfort.
- Participate in Interdisciplinary Team meetings and update the Plan of Care (POC) for assigned patients based on identified problems and needs.
- Ensure patient care is delivered at the appropriate level of care.
- With coordination of PCP and medical director, provide management of medication profile to prevent polypharmacy and ensure optimal symptom management.
- Manage medication profile to ensure hospice is providing medications related to disease process and symptom management.
- Ensure all services are provided in accordance with the patient’s Plan of Care.
- Ensure the POC involves the active participation of patient, family, Primary Care Physician, and the Interdisciplinary Team.
- Provide team members with appropriate information to coordinate all services related to hospice care for assigned patients, including communication and assignments for visiting nurses, home health aide/homemaker, chaplain, social worker, volunteer, and ancillary services, as well as ordering medications and durable medical equipment/supplies.
- Provide consultation and supervision to all CNAs and LPNs involved in patient care.
- Participate in the performance evaluation of clinical members of LPN(s) and CNA(s) involved with the care of the case load.
- Assist with orientation of clinical members participating in the care of the assigned caseload.
- Ensure cost‑effective practices by self and others.
- Assist with QAPI (Quality Assurance and Performance Improvements) activities.
- Work as a team member at all times and uphold the Hospice Code of Ethics.
- Provide care to manage patient symptom and overall comfort.
- Utilize the medication formulary whenever possible for symptom management.
- Triage scheduling and symptom management calls from members of IDT.
- Report to the Clinical Director as patient condition changes or changes in the patient condition that require change in level of care.
The responsibilities of this position involve significant physical activities including standing, lifting, bending, stooping, pushing, pulling, and twisting. The employee must be a licensed driver with a vehicle that is in good working order and insured in accordance with organization requirements.
