This position reports to Director of Home Care. Reviews OASIS and plan of care documentation for accuracy. Helps clear issues. Triages patient phone calls. Helps with remote patient monitoring. In office position
- Milwaukee office. Might cover PM or weekend triage for PTO/Ill calls.
- Graduate of an Accredited School of Nursing. BSN required
- Current WI RN license
- 1-2 years of Med/Surg or Home Care experience
- Current/Valid WI Drivers License; plus reliable/insured vehicle
Monday
- Friday 8am-4:30pm
