Case Manager, EAMH, On Call #1088

Join Life Works NW as a Case Manager for individuals with psychosis. Assist clients in developing independence, advocating for themselves, and connecting with vital resources to reach and maintain stability. Provide services onsite, within the community, and at clients' homes while maintaining accurate and timely records of all activities. This role works with adults experiencing severe and persistent mental illness (SPMI) under the direction of a senior clinician.

Essential Responsibilities
  • Meets standards for documentation and work accuracy as defined in the Performance Evaluation including timely completion of progress notes, assessments, treatment plans, authorization requests.
  • Meets standards as defined in the Performance Evaluation for direct service, implementation and utilization of the clinical model, quality of clinical practice and teamwork.
  • Responsible for client orientation including advising clients and significant other(s) of services offered through the program and recommending services.
  • Provides input on clinical assessments and the development of treatment plans.
  • Provides case-coordination by maintaining contact with other agencies, referral source(s) and staff regarding client and significant other(s) involvement in treatment services.
  • Conducts individual and group skills training sessions based on current evidence‑based practices e.g., collaborative problem solving, cognitive behavioral interventions, and solution focused therapy.
  • Teaches socially acceptable behavior, employing techniques such as behavior modification and positive reinforcement. Encourages individuals and family members to develop and use skills and strategies for confronting problems constructively.
  • Observes and influences clients' behavior, communication and interaction. Detects unusual behavior and reports observations to professional staff.
  • Organizes, supervises, and encourages clients to participate in social, educational, and recreational activities. Provides assistance and support to clients with the job search process.
  • Assists the client to the extent necessary, based on client’s level of functioning, to access needed services and resources. Advocates on client’s behalf. May attend the client’s psychiatric medical appointments, if clinically indicated.
  • Reviews and evaluates clients’ progress in relation to measurable goals described in treatment plans.
  • Provides clients or family members with information about mental health and addictions issues and about available services and programs, making appropriate referrals when necessary.
  • Reports any pertinent information to the proper authorities in cases of endangerment, neglect, or abuse of children, individuals with SPMI or the elderly.
  • Acts as a client advocate to coordinate required services or to resolve emergency problems in crisis situations. Provides clinical support to clients with immediate and urgent needs.
  • Maintains chart notes related to external contracts, obtains pertinent records from other treatment providers, and tracks client involvement, follow‑through and progress in varied treatment activities. Completes documentation in electronic health record system and Service Activity Logs (S/AL's) in a timely manner.
  • Transports clients in personal or agency vehicles and travels between sites or in the community.
Qualifications
  • This position requires a valid and active QMHA credential certification through MHACBO.
  • Bachelor’s degree in Psychology, Counseling, Social Work or a mental health related field OR a minimum of 3 years of prior experience with the related population in the counseling field.
  • Demonstrated ability to communicate effectively; understand mental health assessment, treatment and service terminology and individual, group, family and other counseling techniques; program policies and procedures for services and supports identified in an Individual Services and Supports Plan.
