Job Overview

Location: UMMC West Bank

Hours: 64 hours bi-weekly

Schedule: day/evening/night, every third weekend

This position will provide clinical support and distributive services in coordination with physician and nursing services to deliver quality and cost-effective drug therapy to patients. Provide pharmaceutical care for patients, monitor drug therapy and coordinate drug distribution, ensure medication safety, participate in education of students and/or residents, and develop and maintain professional relationships with other healthcare providers.

  • Provide pharmaceutical care to hospital patients as a member of the pediatric pharmacist team
  • Review new medication orders for appropriateness of therapy: including patient assessment, identification and documentation of drug therapy problems and resolutions, review of appropriateness of therapy and follow-up and discharge counseling for selected patients
  • Monitor patients on an ongoing basis and adjust medications and doses after consulting with the prescriber
  • Provide interpretation and problem solving of physician’s orders, checking for formulary status, appropriate drug selection, dose and dosage form, drug-drug interactions, drug-nutrient interactions, drug duplication, potential for adverse events, appropriate route of administration, therapeutic substitution, IV to PO conversion, or other formulary or departmental initiatives
  • Communicate with physician or nurse to insure prompt clarification on any part of the order in question, and recommend alternatives that are on formulary, if non-formulary medication is prescribed.
  • Evaluate therapy for optimal patient outcomes while ensuring medication safety
  • Coordinate drug distribution in assigned areas.
  • Complete or review entry of medication orders in the pharmacy computer system
  • Prepare/dispense medications, sterile products and compound IV admixtures and other extemporaneous orders as requested using good technique and safe practices
  • Perform medication storage area inspections as assigned
  • Direct and oversee the drug distribution activities of pharmacy technicians and students
  • Provide indirect supervision to technician staff
  • Participate in team meetings/discussions with clinical pharmacy specialists, pharmacy residents, pharmacy students and technicians
  • Foster an environment of teamwork with other department staff and customers
  • Serve as a preceptor for students and/or pharmacy residents on rotation
  • Provide training and education to other staff members
  • Provide customer service focused team approach to internal and external customers
  • Provide drug information and consultation to physicians, nurses, patients, and other healthcare professionals.
  • Identify customers’ learning needs
  • Consult with and educate customers in an effective manner
  • Develop and maintain interdepartmental relationships
  • Act as a primary resource for nursing staff on the proper administration of drugs
  • Provide training and education to others as necessary
  • Remain current with new drug information for patient populations served
  • Respond to pediatric code blue emergencies as assigned to assist with drug preparation and information on emergency medications intervening as appropriate to ensure optimal patient outcomes.
  • Participate in Medication Safety, ADE, DUE and performance improvement activities
  • Complete medication safety and ADE forms when issues are identified
  • Complete and follow through on Drug Usage Evaluation (DUE) forms as assigned
  • Identify additional opportunities to improve patient care and the performance of the pharmacy department
  • Meet all role-related and age specific competencies. Comply with all initial and ongoing competencies within the entity and department. Complete required learning relevant to the role.

    D or Bachelors in Pharmacy
  • Eligible for MN Pharmacy License
  • Completion of an ASHP-accredited PGY1 residency program
Preferred
  • Completion of a PGY2 Pediatric Pharmacy Residency or equivalent pediatric experience
