Provide Exceptional Care—Join Southside Community Health Services as an Advanced Practice Provider (NP/PA)
Southside Community Health Services (SCHS) is a full-service, non-profit healthcare clinic serving the diverse South Minneapolis community. Our mission is to improve the health of our patients and communities by delivering exceptional care, removing barriers, and promoting healthy lifestyles. We are seeking an Advanced Practice Provider (NP/PA) to provide high-quality, patient-centered care as part of our integrated healthcare team.
About the RoleThe Advanced Practice Provider provides comprehensive, patient- and family-centered care within their scope of practice. This role works collaboratively with medical and interdisciplinary care teams to support patients' health goals and deliver accessible, trauma-informed care to our diverse community.
Key Responsibilities- Provide comprehensive patient- and family-centered care, including health maintenance, preventive exams, acute and episodic care, chronic illness management, and patient education
- Collaborate with other health professionals to coordinate patient care and achieve health goals
- Complete full patient contact hours based on FTE
- Participate in rotating on‑call duties, as applicable
- Complete disability, FMLA, workers compensation, and other required forms and letters
- Maintain timely documentation, including closing charts and addressing Epic in‑basket items within 48 hours
- Complete peer reviews and maintain current credentialing and licensure/certification requirements
- Follow SCHS policies, clinical protocols, and quality assurance requirements
- Complete assigned Relias courses, policy attestations, incident reporting, and other administrative responsibilities
- Participate in staff meetings, committees, and organizational activities
- Assist with onboarding new employees and supervise/precept students in the clinic
- Assist with other duties and responsibilities as assigned
- Commitment to serving diverse and underserved populations within an integrated healthcare environment
- Strong community health/public health orientation

- Strong clinical judgment and experience providing direct outpatient care
- Ability to work effectively with minimal supervision and as part of a multidisciplinary team
- Strong analytical, problem‑solving, organizational, and time management skills
