This is a grant-funded role.

We are currently seeking a Mental Health Associate to join our Collaborative Care Model team as a Behavioral Health Clinician (BHC). This is a grant-funded position backed by the Substance Abuse and Mental Health Services Administration (SAMHSA), in collaboration with the Minnesota Department of Human Services. The Collaborative Care Model (CoCM) is a specific integrated behavioral health model, and is widely agreed upon as the integrated care approach with the most robust evidence base proving its effectiveness.

Our team at Hennepin Healthcare is leading Minnesota in its implementation. It is a team-based approach to treating mental health conditions – especially commons ones like depression and anxiety – within primary care settings.

The Behavioral Health Clinician (BHC) is a core member of the CoCM team, facilitating communication among all team members and acting as the lead contact person for the patient.

This position will work with adults in our downtown primary care clinic. Applicants should feel comfortable working with a growing program and willing to share new ideas with leadership to help with continuous improvement efforts and long-term sustainability.

The SAMHSA grant that funds this position is set to run through September 30, 2028, but efforts are underway to continue this work into the future.
