Primary Responsibilities:
- Perform weekend Start of Care (SOC) admissions with complete OASIS documentation
- Develop and initiate care plans in collaboration with the interdisciplinary team
- Evaluate patient needs and coordinate necessary services
- Educate patients and caregivers on disease management and home care best practices
Required Qualifications:
- Valid Minnesota RN license and CPR certification
- Proven experience in home health and OASIS documentation
- Reliable transportation, driver's license, and active insurance
- Strong communication and clinical assessment skills
