TLC Nursing Associates, Inc.
TLC Nursing Associates, Inc. is seeking a skilled and compassionate Registered Nurse (RN) to provide high-quality care to patients in a home health setting. The ideal candidate will deliver personalized nursing services, monitor patient progress, and ensure continuity of care in collaboration with physicians and interdisciplinary teams.
Job Responsibilities
- Conduct in-home assessments to evaluate patient health status and develop individualized care plans.
- Administer medications, wound care, and other treatments as prescribed by the physician.
- Monitor patient conditions and update care plans based on changes in health status.
- Provide education to patients and caregivers on disease management, medications, and safety practices.
- Document all nursing activities accurately and in a timely manner according to agency and regulatory standards.
- Coordinate care with physicians, therapists, social workers, and other home health staff to ensure optimal outcomes.
- Follow infection control procedures and maintain a safe care environment in the home.
- Active and unrestricted RN license in the applicable state.
- Minimum of 1-2 years of clinical nursing experience, preferably in home health or community-based care.
- Current Basic Life Support (BLS) certification.
- Strong assessment and critical-thinking skills with a patient-centered approach.
- Ability to work independently in home settings and manage time efficiently.
- Excellent communication and documentation skills.
