The Minnesota Department of Veterans Affairs is looking for a part‑time Nurse Practitioner to provide primary care to residents residing at the Minnesota Veterans Homes. The nurse practitioner in this position will perform regulatory visits for up to 72 Veterans and their spouses at the Minnesota Veterans Home – Montevideo and a smaller number of residents at the Minnesota Veterans Home – Minneapolis.

In addition, the incumbent will be expected to perform in‑person assessments on and coordinate care for residents when physically on site for sub‑acute issues as clinically appropriate. Because of the distance between the facilities, supervision of medical care at both MVH sites will be shared with physicians assigned to each facility. The incumbent will work as a member of the multidisciplinary team at each facility and as a member of a Minnesota Veterans Homes Medical Clinic Group Practice consisting of 3 physicians, 3 nurse practitioners and a medical director.

This position is designated as part‑time, 0.8 FTE and the incumbent will work four days per week with the weekdays worked to be determined in coordination with the physician providers. The incumbent will also have after‑hours/weekend on‑call…
