Job Description

We're hiring a motivated and compassionate Physical Therapist (PT) for a contract assignment in a Skilled Nursing Facility (SNF) setting located in Lamberton, Minnesota. This position includes scheduled floating to Worthington, MN two days per week.

The ideal candidate is a dependable Travel PT or Contract Physical Therapist who enjoys working in a collaborative rehab environment and providing high-quality patient care to an older adult population.

Responsibilities

• Evaluate, plan, and deliver physical therapy treatments to patients in SNF and sub-acute rehab settings.

• Create individualized treatment plans to help residents improve mobility, strength, and functional independence.

• Accurately document all therapy sessions, progress notes, and discharge plans per facility and regulatory requirements.

• Work closely with occupational therapists, speech-language pathologists, nursing staff, and physicians to coordinate patient care.

• Maintain an 85% productivity standard.

• Travel between Lamberton and Worthington facilities as scheduled.

Qualifications

• Current and valid Physical Therapist (PT) license in the state of Minnesota (or eligibility for MN licensure).

• Graduate of an accredited Physical Therapy program.

• Experience in a Skilled Nursing Facility (SNF) or geriatric rehab preferred.

• Excellent communication, organization, and time management skills.

• Reliable transportation for float days.
