RN Visit Nurse Overview

As an integral member of our team, you'll:

• Perform comprehensive assessments to create and update nursing diagnoses and plans of care for home health patients.
• Administer skilled nursing procedures such as wound care, medication management, assessments, and treatments as ordered by physicians.
• Educate patients and their families on healthcare topics and self-care management to promote optimal wellness.
• Collaborate with multidisciplinary teams to coordinate overall plans of care and ensure holistic patient support.
• Maintain compliance with healthcare regulations, standards, and ethics while documenting assessments and treatments accurately.
• Participate in continuing education to stay current and maintain professional certification, adapting quickly to changes in the healthcare landscape.

Qualifications

To excel in this role, you'll need:

• A current Registered Nurse license in the state of Minnesota, along with a valid Driver's License and reliable vehicle.
• Knowledge of nursing theory and practice, strong organizational and interpersonal skills, and the ability to adapt quickly to changes and prioritize multiple tasks.
• Experience with Electronic Medical Records (EMR) software is preferred, and prior nursing experience in a home health, hospice, or TCU setting is ideal.
