The Occupational Therapy Assistant (OTA) provides occupational therapy services to clients in their homes in accordance with the Plan of Care and the Company's policies and procedures, under the direction and supervision of a licensed Occupational Therapist. The OTA reports directly to the Registered Occupational Therapist.
Base Pay Range$24.00/hr – $26.00/hr
ScheduleThis is a full‑time position, Monday – Friday with an on‑call rotation. The role requires travel to clients' homes to provide 1:1 client care that may take 1–3 hours per client.
Coverage AreaThis position reports to the Buffalo, MN office and provides services in Elk River, Becker, Big Lake, Zimmerman, Princeton, Monticello and surrounding cities.

- Provide treatments to restore and enhance the physical and psychosocial function of clients under the direction and supervision of a licensed Occupational Therapist.
- Carry out occupational therapy maintenance programs developed by the licensed Occupational Therapist.
- Demonstrate therapy program for home health aide, family member, or other responsible person who may assist the client with the home occupational therapy program as directed by the licensed Occupational Therapist.
- Reinforce teaching of home therapy program to client, family, or caregivers as directed by the licensed Occupational Therapist.
- Maintain appropriate documentation for the clinical record as required by Company policy and procedure.
- Confere with the licensed Occupational Therapist regarding client progress and Plan of Care changes.
- Participate in case conference and in‑service education.
- Maintain positive and effective communication with all staff and others.
- Participate in the team concept and maintain absolute confidentiality of all information pertaining to clients, families, and employees.
- Occupational Therapy Assistants cannot perform the following activities: evaluation, treatment planning or establishing plans of care, change of treatment, supervision, designing, fabricating, and fitting orthotic devices.
Heavy to very heavy. Involves lifting clients, bending, stooping, stretching, and setting up equipment. Must adapt to various environments and community settings. Must be able to drive or use public transportation in all weather. Exerts in excess of 100 pounds of force occasionally, in excess of 50 pounds of force frequently, and in excess of 20 pounds of force constantly to move objects.
Physical demand requirements exceed those for heavy work.
- Graduate of a two‑year COTA college program leading to an associate degree, approved by the American Occupational Therapy Association, and currently licensed as an Occupational Therapy Assistant in the state of Minnesota. The license must not be revoked, suspended, or restricted.
- Have at least two years experience in a healthcare setting or equivalent experience. Home care experience preferred.
- Strong interpersonal communication and teaching skills.
- Strong organizational skills and the ability to work independently.
- Excellent written and oral communication skills.
- Access to a dependable vehicle or public transportation to travel to multiple company business stops a day. A valid driver’s license and proof of car insurance when using a personal vehicle for company business.
- Proof of negative Mantoux or documentation of negative chest X‑ray.
- Current CPR certification recommended.
- U.S. Citizenship or evidence of a valid Alien Work Permit.
- Disclose any conviction and criminal history records pertaining to any crime related to the provision of health services. Candidates with such convictions will not be hired.
- Pass initial and ongoing background studies and screenings, including those of the Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities.
Adara Home Health, Inc. is committed to fair and equitable compensation practices. The base salary range for this position is $24 – $26 per hour, determined by relevant work experience, skills, certifications, job level, supervisory status, and location.
