Job Description

We're hiring an experienced and motivated Occupational Therapist (OT) for a Skilled Nursing Facility (SNF) contract position in Lamberton, Minnesota. This role includes floating to a partner SNF in Worthington, MN two days each week. The ideal candidate is a dedicated rehab professional who values patient-centered care and thrives in a supportive, team-based environment.

This is an excellent opportunity for a Travel OT or Contract Occupational Therapist looking for consistent hours, a great facility, and the flexibility of a contract position.

Responsibilities

• Provide high-quality occupational therapy services to patients in SNF and sub-acute rehab settings.

• Evaluate patient abilities, develop treatment plans, and deliver individualized therapy sessions.

• Document all care and progress according to state, federal, and facility guidelines.

• Collaborate with a multidisciplinary team including PTs, SLPs, nurses, and case managers.

• Maintain 85% productivity across both assigned locations.

• Travel between Lamberton and Worthington facilities as scheduled.

Qualifications

• Licensed Occupational Therapist (OT) in the state of Minnesota (or eligibility for MN licensure).

• Graduate of an accredited Occupational Therapy program.

• Previous experience in a Skilled Nursing Facility (SNF), long-term care, or geriatric rehab strongly preferred.

• Excellent communication and documentation skills.

• Reliable transportation for scheduled float days.
