Talented Medical Solutions is seeking a dependable and empathetic Licensed Practical Nurse (LPN) for a Geriatric/Extended Care (GTE) contract assignment in Fertile, Minnesota. As an LPN in a GTE setting, you’ll care for residents who require ongoing support with dignity, respect, and a personal touch. Your role will be key in maintaining quality of life and providing consistent, skilled nursing care.

Key Responsibilities:

Administer medications and treatments as directed

Monitor resident conditions and report changes to supervising staff

Assist with ADLs and daily personal care

Document care and follow facility protocols

Collaborate with RNs, providers, and support staff to ensure safe, coordinated care

Active Minnesota LPN license (or eligibility to obtain)

Current BLS certification

At least 1 year of experience in geriatrics, long-term care, or similar settings preferred

Strong interpersonal skills and a commitment to patient-centered care
