Job Title: RN Case Manager – Lead with Compassion

Pay: $90,000 – $130,000 DOE

Benefits: Full Benefits Package

• Develop and implement individualized nursing care plans
• Monitor patients’ comfort, pain, and spiritual well‑being
• Act as liaison between patients, families, and providers
• Address crises with calm, clear judgment
• Coordinate with the care team to ensure consistent, quality outcomes

• Licensed RN in Minnesota
• Experience in hospice, oncology, or chronic illness management
• Strong communication, problem‑solving, and organizational skills
• Commitment to upholding dignity, autonomy, and comfort
