Every Other Weekend and Holiday Rotation Required.

Active MN CNA required at the time of hire. CNA certification must be obtained within four months of hire; training is provided by the facility.

Pay: $23.51–$28.69 per hour. Sign‑On Bonus: $10,000.

Job Summary

The Nursing Assistant (NA) serves as caregiver to the resident during the scheduled work period in long‑term care. Provides resident‑centered nursing care and daily living assistance to assigned resident under the supervision of a registered nurse (RN). Knowledge of and delivery of age‑appropriate care related to the physical and psychological needs of the resident as per care plan. The NA is expected to follow infection prevention and control policies and PPE use.

Assists with basic health‑care needs including bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support and other personal care to assigned resident. Helps the resident in transferring, repositioning, and walking using correct transfer techniques and equipment, and also provides range‑of‑motion and passive exercises. Completes tasks related to food preparation and dining, while creating a positive dining experience for the resident.

Assists residents with meals and snacks, provides water and supplements as appropriate.

The NA aids providers and nursing staff with procedures as needed. Documents resident interactions and outcomes related to resident care, behavior, activity, and diet. Communicates resident's changing condition and related concerns to the charge nurse. Collects non‑invasive body fluid specimens and gathers vital signs. Provides housekeeping and laundry services to assist residents with keeping living areas clean and operational. Provides assistance and documentation with wellness and recreational programs, encouraging participation.

May participate in departmental/organization committees and task forces as needed.

Qualifications
  • High school graduate or GED preferred (not required).
  • Minimum age 16.
  • Prior clinical or nursing assistant experience in long‑term care preferred.
  • Certified Nursing Assistant (CNA) certification required with the State Board of Nursing, or state‑certified CNA within four months of hire, or enrolled in a training program or endorsement.
  • Must satisfactorily complete yearly department‑specific competency testing and maintain a record of continuing education credits; a minimum of 12 hours continuing education required annually.
  • Maintain all department‑specific required certifications.
  • When applicable, be certified and registered with the Unlicensed Assistive Person (UAP) Registry.
  • Depending on facility requirements, may classify as a Sanford Category II Driver (requires valid driver’s license, motor‑vehicle report, and medical qualification).
