- Develop and implement individualized nursing care plans
- Monitor patients’ comfort, pain, and spiritual well‑being
- Act as liaison between patients, families, and providers

- Coordinate with the care team to ensure consistent, quality outcomes
Ideal Candidate Profile:
- Licensed RN in Minnesota
- Experience in hospice, oncology, or chronic illness management
- Strong communication, problem‑solving, and organizational skills
- Commitment to upholding dignity, autonomy, and comfort
