• Location Hutchinson MN 55350
• Hours 12 hour Shifts
• Days Every other Thursday Saturday & Sundays
• Overnights Every Tuesday Wednesday & Thursdays
• Client Specifics 20 year old with a trach vent and a g tube

• Provide skilled nursing care in home and school settings
• Assess and monitor your patients condition and communicate changes with the care team
• Administer medications treatments and prescribed therapies
• Provide trach ventilator airway and G tube care as needed
• Maintain accurate documentation and support families through education
• Follow individualized care plans and maintain a safe care environment
