Moments Hospice is seeking an experienced RN Case Manager to serve as the clinical lead for patients and families, manage symptoms, coordinate care, and provide guidance through end-of-life care.
This full-time, field-based role features a Monday–Friday schedule (8:30 AM–5:00 PM) with a salary range of $75,000–$90,000 per year, plus a fleet car program and comprehensive benefits. You'll work in a supportive environment with access to leadership and real opportunities to shape your career.
